Trunk or Treat Trunk Registration Form

1) This form and all paperwork needs to be turned into Livingston Recreation at Livingston City Hall by Friday, October 28, 2022 by 5pm. You will receive confirmation with Participant Rules when form is turned in For multiple entries, please submit one (1) form per vehicle / Proof of valid registration and insurance must be provided for each vehicle 2)I do hereby fully release and discharge the CITY OF LIVINGSTON its officials, officers, volunteers and employees from any and all claims from injuries, damage or loss, which I may have or which may occur to me as a result of my participation or attendance in any of the CITY OF LIVINGSTON programs, activities and/or event and allow the CITY OF LIVINGSTON to use and/or televise in any manner, my picture and/or likeness and to indemnify and hold harmless the CITY OF LIVINGSTON and it’s officials, officers, volunteers and employees from any and all claims, damages and/or loss of any kind. I ALSO CERTIFY THAT I am a licensed driver and the vehicle I will use has current vehicle registration and vehicle insurance. I also agree to follow the rules for this event.
